*Credentialing: $250 per provider per payer for government insurances and $150 per provider per payer for commercial insurances will be charged separately.
*ZMB will charge 50% of the credentialing fee upon execution of the contract, prior to submission, with the remaining 50% due upon successful completion of the credentialing process with the applicable payer(s).
*Paper Statements: $1.00 will be charged for each paper statement sent to patient.
